Be sure to completely fill out each questionnaire you receive and/or asked to fill out online. You are advised to ask the AZU staff for help in attaining player stats like approach jump, standing reach and block jump if you do not have those stats already.

After completing the questionnaire it is important you realize that many student-athletes received the same questionnaire and you are one of many prospective student-athletes for which they have started a file. This is said not to discourage you, but to help you realize that the questionnaire is at the beginning of a lengthy process for college coaches and you.
